Helen Grus, a detective with the Ottawa Police Service’s SACA unit, tells the Allison Inquiry that in January 2022 a colleague approached her with concerns about what appeared to be a doubling or tripling of unexplained infant deaths. Before a town hall meeting with then-Police Chief Peter Sloley, she says she queried the department’s records management system and identified six confirmed unexplained cases — up from a typical two to three per year. She says she raised the data with the chief, while being explicit that she was not drawing conclusions about the cause. She also describes finding that some case files lacked a completed SUDI questionnaire (sudden unexpected death in infancy), and that one file had been closed within 24 hours of the infant’s death, before any autopsy results were received.
